The Korean oriental medicine doctor dies of a car accident, and wakes up in the body of a crown prince that has only 90 days to live. He uses acupuncture to help improve the condition of his body a bit, and this unlocks his hidden bloodline skill. When he helps sick people, he receives a reward to extend his life by a few days. The way his skill works is complicated to explain in a single sentence. When he cures a sick patient, his hidden class automatically calculates the amount of life that had been extended in his patient. Then the hidden class does some kind of math calculation and adds the proper amount of days to his own life. In the original story that he isekai into, after the crown prince dies, a domino effect of events happen and war havocs the world. His brother, who has a soft heart, becomes the core reason of the downfall of his kingdom. So in addition to helping sick people to extend his own life, he has to tackle on the job of preventing the downfall of his own kingdom.
